Due to growth in our services the At Home team of professionals is looking for a Care Coordinator to service our clients in the South Region. Designed to make a genuine difference in the lives of older people, you will ensure quality care is delivered to our clients.
Reporting to the Client Services Manager you will support clients by conducting assessments, reassessments and service coordination in line with the organisation’s strategy, Christian Vision, Mission and Values and operating requirements.
To be successful in the role, you will possess previous experience in community-based/aged care service coordination, strong customer service skills, demonstrated ability to prioritise and delegate tasks, administrative/office environment experience and excellent phone manner. A superior communicator, with the ability to communicate effectively to all levels of staff and management.
